Not sure if they still do, but I joined online 2 years ago and it was always the case that you had to go instore ask for it, they didn't just send them out. So, worth popping into a store and asking

Ah Chelsea26 that's very kind! Don't worry though, I will just buy a cheap one. Mothercare has a cheap one I might get. I just heard that the Boors one was quite good for free.

Re what happens with the parenting club- it's just added to your membership, so you carry on using your old boots card, but automatically get 10points for every £ on most baby items.
Once a quarter they send out a leaflet with some discount vouchers.
