Incorrect follow u letter

Hi I had cancer and am now on a 10 year follow up plan. Every year I have a blood test followed by a ultrasound.
I had my blood test last month and I received a letter from the nurse stating she had done bloods and a ultrasound and that both were ok but she will check with the consultant. I then received another letter stating the consultant agreed everything looked fine.
I haven't had an ultrasound so not sure where she is getting this in formation from. I have a telephone consultation on Tuesday but it doesn't fill me with confidence that they can get something so simple wrong.

It’s just an administration error, maybe the nurse used a letter template and forgot to delete the ultrasound bit after she used the template for a previous patient?
It’s not great, and it absolutely shouldn’t happen, but unfortunately things like this do happen occasionally.
It isn’t a reflection on the care you have or will receive in the future.
I imagine when you discuss it during your telephone consultation you will get an apology and an arrangement for the ultrasound.
