I need to loose weight naturally for an upcoming operation , I am really struggling. Could I diet every other day , whilst eating normal in-between? I have about a 20lb to loose ?

You could, if you still ended up in a sufficient calorie deficit across the week. Would it not be easier to average that out, instead of lower calories one day and maintenance calories the next? So for example, instead of 1200 cals one day, and 1800 the next, just have 1500 each day?

How long till you need to be at target weight?

20 lbs might be a difficult target to achieve in 7 weeks. A regular 2 lbs a week would still leave you 6 lbs off target. I guess losing 14 lbs is still better than not losing 14 lbs though?

3 lbs a week? You will have to go quite low every day. And move with purpose, lacing up for shoes and going for an hour long walk every morning.
It can be done. Make vegetables the bulk of your plate. As long as you don't add fat, you can eat as many vegetables as you want. Green beans, tomatoes, sliced peppers, fennel ...Eat chicken or poached fish, bin all the snacks in the house, office, car.

You have a purpose. You need that weight to be gone for the operation.

What are you doing that doesn't work? What did you eat and drink yesterday.
