Pulling ear while breast feeding

DD is 4 months old and has been EBF. Over the past week or so she has been rubbing her fist and sometimes fingers into her ear when she feeds. She only does this when she feeds from the right side i.e. her left ear, and only really during the day as I think she's too sleepy during night feeds to do anything else apart from feed.

So - any ideas why she's doing this? I'd like her to stop as she's cut her ear a couple of times already from her sharp little nails, and I don't want her to make it really sore. I've tried holding her hand out of the way, but that annoys her and interrupts her feeding.

DS does this as well. If he has been sucking/chewing his fingers then it makes his ear all red and sore looking I put a tiny bit of nappy rash cream on it when he is asleep and it always seems fine by the time he wakes up. I haven't really tried to discourage it as it's a little comfort thing for him - I just keep his nails short to minimise any risk of him damaging anything.
