Is it really likely to get from there into your breastmilk?

Usually, topical meds are not systemic and for this reason, would be safe with bf.

This may not always be the case, though, so don't quote me

Probably it's just never been specifically tested on regnant or breastfeeding women so they are doing the standard legal arse-covering (hah! see what I did there?). I think I would risk the chance of a spontaneous rectal-nippular pathway spontaneously establishing itself, personally [disclaimer: but I have no medical qualifications whatsoever].
