Could it be reflux intermittently?

Can reflux be intermittent? I have a 5 week old and he sometimes seems to show reflux symptoms and other times seems really happy and content!

He has a wheeze when he sleeps on his back which seems worse if I haven't got all the burps out
He often pulls on and off whilst feeding
He cries a fair amount and when he does he arches his back and goes very red
He randomly spits back up milk sometimes after feeding sometimes a while after
His naps are irregular in length.. sometimes 30 mins sometimes an hour and a half
In terms of his sleeping, he generally tends to do 2x two and a half hour sleeps and then from 4am is very fussy and I struggle to get him to sleep longer than 45 mins at a time before he wakes up crying, he is very happy to sleep upright on my knees (obviously not when I'm sleeping)
He is putting on weight really well which is why I haven't been too concerned but the last few days he has been more and more unsettled and bringing milk back up (both what looks like fresh milk and digested milk) and I also wondered if it was reflux as he does sleep part of the night ok?! I try to keep him upright after feeding as much as possible but this doesn't always seem to make a huge amount of difference!

I have contacted my health visitor but posting here in case anyone has experienced similar.

My daughters reflux at 5 weeks was intermittent too. The 4 am feed was the worst feed. Then she started being bad during the day but better at night. From 12 weeks she was unbearable and screamed all day and woke throughout the night. She has mild -moderate reflux, severe silent reflux and multiple food allergies. As her reflux was mostly silent she gained weight ok. She was happily on the 9th percentile until I stopped dairy then moved up to the 25th-50th percentile so even though her weight gain looked ok as she was following her line and I'm only small her weight gain was impacted
