Constipated on reflux milk

My little one has been on reflux milk for the last 3/4 months she’s now nearly 5 months old she really struggles to poo she’s straining for a good 30/45 minutes just for there to be a little hard poo in her nappy I was worried so phoned the GP I was prescribed lactulose and was told 2.5 ml twice a day but was told that as soon as her poo returns to normal to stop using it as it will make her bowels lazy. Even tho I explained that surly because her milk is thicker (which causes constipation) she will always need it for as long as she’s on thicker milk but she reassured me that it shouldn’t be the case hopefully.
After a week or 2 on the lactulose her poo went normal again so I stopped it but now she’s gone back to struggling again but this time it’s worse.
I feel so helpless. I’ve tried the basic thing like giving cool boiled water, rubbing her belly and moving her legs.
Is there anything else I can give her to help or anything I can ask the GP for that will actually help her without causing damage in the long run.
thank you if you’ve read this far I know it’s long 🙂

Can you speak to the gp agsain? Maybe she needs to continue with lactulose until weaning where you can add more fruit in
