Its a nipple thing .... advice please

Can anyone tell me what the creamy white lumps on the end of my nipple could be. They are sort of hard, not blisters, just off centre and do soften in the bath but are sore to touch, making feeding painful. They come and go.

l already know l have thrush and am being treated with fluconazole along with my 11 week old DD. My nipples sometimes look white and bloodless on the ends, remaining hard and painful with after pains following a feed, which l had thought was connected to thrush.
