Has been favouring the right for about a week and is now completely refusing to feed from the left, screams when I try, as a result my left breast is about to explode - any suggestions?

Have you tried feeding him in different positions? When dd2 did this, I would give her the breast she didn't like when she was too sleepy to complain.

He'll do it in the night but has been screaming most of the day, have fed him from the right in a more upright position and he'll eventually feed with that but is refusing the left completely - think I may have to express from my left but am not sure how.....don't have a pump

What if you lie him down on the bed on his right side, then you lie with your head at his foot end, so that he can feed from your left - might just change the pressure on his gums? If it's any comfort, this is very likely to be a short phase.
I have expressed by hand but only smallish quantities for adding to food and it made my thumbs ache. Not sure you could do it if you're engorged though. Wearing a cabbage leaf inside your bra may make you feel more comfortable.
