Falling asleep on the boob

So my nearly 5 month old can't sleep without a boob in her mouth :( I've tried rousing her after feeds just before I put her down to sleep but she totally freaks out.

Does anyone have any experience of successfully breaking the habit?

I'm on my second night of pick-up, put down shush pat type Baby Whisperer thing and it is breaking my heart!

Try the No Cry Sleep Solution - Pantley Pull Off method. Though I tried that with DC1 and actually found PUPD worked better for him - we started by swapping the bedtime routine around though so I fed before bath time and then DP rocked to sleep. Once he could fall asleep being rocked then we did the shushpat PUPD thing.
