Cmpi?-5 wk old screaming in pain trying to poo for 24 hours

My 5 wk old is struggling every time he poos. The whole process takes at least 24 hrs of him having spasms of pain - crying, bringing legs up, writhing around and even crying to try and fart. His tummy is making loud gurgling sounds. He has already been dx with reflux and is on v high dose ranitidine. I am bf exclusively and fanatically avoiding dairy and soya. I am wondering if he still has a problem with my breast milk? Dd and ds ended up on neocate for reflux and paed gastro specialist said they had cmpi. But no testing ever done. I am so reluctsnt to give up on bf but i hate seeing ds2 in pain and obviously eant to do the best thing for him. Anyone think the bowel symptoms could be to do with my milk? Dont want to cut anything else from diet as have tried this before and didnt help much and ran out of milk.

Not v much any more since reflux undrr more control although i can still hear it coming halfway up

Ranitadine has constipation as a side effect - could it be this?

If you've cut out all dairy and soya the surely it isn't CMPI? Some tinies do get issues with pooing, mine did and the doctor said they call it 'grunting baby syndrome', where they haven't yet worked out how to relax their bottoms while pushing down, resulting in the poor little things essentially hammering on a tightly closed trapdoor Confused He got much better by 10 weeks, until then we just gave him warm baths, tummy massage and when his constipation got really bad, 30ml of cooled boiled water on the GP's advice, which sorted it out.

Fwiw, DS had (and has) very severe reflux and I did full elimination diet for 4 weeks, 2 weeks of which he was exclusively on Elecare hydrolysed formula (like neocate) and it made no difference to his gas and wind. We also suspected that his meds could be the culprit, but it seems that time was actually the most important factor!

thanks sunnysummer, really useful thank you. i am slightly sleep deprived so not explaining properly, i meant that could he be allergic to my breast milk. i know this is rare but dd and ds1 were according to the paediatric gastro-oesophageal consultant. he never tested though, it was by process of elimination, also, i ran out of milk with both of them so it didn't really matter.
ds2 has had a much better 48 hours and pood ok yesterday although grunting again this am. you've given me hope that it could be the same as your lo. my only other concern is that i've just discovered that the domperidone i am taking for milk prod has lactose in. wondering if that would effect him? it is literally only dairy in his diet.

Lactose is in breastmilk - so lactose in your diet makes no difference.

I see your baby is on a 'very high' dose of ranitadine - more likely to be side effects from that, yes?
