He's breastfed but has a bottle of expressed at night, recently seemed to take a lot more effort to get it out. Does anyone know how to gauge the number for him?

Probably just trial and error, buy a couple of different sizes, see how he gets on, my dts still have a bottle at night, they are 2.5years, but will only dring with a newborn teat.

my ds is 15 months and has a medium flow teat

If using MAM for example you could change from no 1 newborn to no 2 baby fast flow for example. A lot easier for them when they are older.

I would buy the next one up from the one you are using and give it a try. You will soon see if it is too fast and can then just stick it in the cupboard for a while.
