Refusing one breast...again

DS is now 6 months. He frequently goes through periods of refusing one breast and we're there again now. On previous occasions it's resulted in blocked ducts or mastitis.

He's always been very 'particular' and has always refused to take the left breast first. It is always his second choice and for the last 48 hours he hasn't used it at all. It's usually the side he falls asleep on, but he's been taking one suck (if he'll even accept lying on that side), coming of and sticking his thumb in his mouth to sleep.

I've been hand expressing to relieve the discomfort. Is there anything else I should do? His night time sleep is really poor at the moment (waking 1-2 hourly) and I wonder if this is related (not taking a full feed).

Any advice welcome - thanks.

Not really sure on this one, what positions have you tried? Have you also called one of the bfing helplines?
