Advice on weaning off the breast

DD is now 11m and has been exclusively bf since birth (though obviously now eating solids before you all think the poor thing is starved!). She has slept through the night in the past, but at the moment is still waking for a feed at 3/4am and is starving. She then goes back to bed and sleeps till 7/8am but is consequently not hungry for breakfast, has a little lunch, milk mid afternoon, dinner and milk again pre bedtime (which usually sends her to sleep, often at the breast). I have never managed to get dh to do any of the feeds, and if I'm honest, a good night's sleep and a bit of independence are part of the reasons for wanting to wean her, though I will definitely miss the closeness. Just wanted some tips on how to wean her off the breast - she does take a bottle and can use a cup - and how to "tough out" the getting to sleep/ waking in the night bit which I guess will be a major change for her
