I think she genuinely hasn't a clue. Waited this long because of previous issues.

Maybe something like 'Darling, you're going to have a little sister or brother soon' - and list all the exciting things, but don't lead her to believe she will be able to play with it. (I remember my acute disappointment at four when the baby arrived and was not an instant playmate, and the tensions this caused.)
