Don’t understand a MAR chart, anyone help?

Hi there
i would like some carers and nurses to help me understand and annotate a MAR chart for me. It’s only for one week but I’m not understanding how it works. If anyone could help me I’d be very grateful. I would obviously show without any names etc to protect gdpr x

Are you administering the meds?

Is there anyone who could explain.

Lostmum2715 · 25/07/2024 15:17

No not me. It’s a mar chart copy of meds my mum had before she died.

What is your question? Why can't you ask the person who completed it?

I'm very sorry for your loss OP xx

Could you contact the care company/nurses? What's the question?

If you can post a picture and what you want to know then I can take a look, I administer medication in a care home setting.

Thanks guys. I had to fight to even get the mar chart so they refuse to speak. Could you possibly interpret how many tablets were taken on each day and times? x

Looks to me that on the 14th she had 1 tablet at 0800 and 1 at 1800, and the rest of the days she 3 tablets in total 1 at 0800, 1800 and (I think) 2400.

The notes in between the times are the number of tablets left in the packet. You can see it counting down.

The PRN bit means 'when necessary' so it wasn't something that necessarily had to be administered each and every time and presumably wasn't in the medication tray with other meds that were administered routinely.

as it's codeine she was probably asked if she wanted it for extra pain relief on each meds visit and if she said yes they would administer it and mark it on the MAR (or if she wasn't able to say they would use judgement if they felt it was needed).
