writing a living will template

I wonder if any of you know of where I might find a template for this kind of thing - last wishes and so on. I have offered to find something like this for my elderly father, to print out for him to fill in, as I don't know what he would want and he is convinced he is not long for this world...I'm not comfortable having a detailed face-to-face conversation about it...for a long list of reasons. Could anyone help at all in suggesting whether there is anything useful on line or whether I have to contact funeral directors directly, though I don't want him to get involved in any prepayment packages etc.

there are plenty of legal websites with free advice on these too

or buy in post office for about £10

Mine came from the Voluntary Euthanasia Society, but it has a different name now.
