4yo tantrumming and refusing to walk/running away/running toward road

Basically the title...what to do?? This afternoon we spent maybe 45 minutes on the same stretch of road, her lying on the ground screaming or running toward the busy road and having to be restrained and pulled back. She doesn't use a buggy anymore and can get out of the straps anyway.

It can last for hours until she tires herself out. Nothing works - not threats, bribes, shouting, kind words, saying I'm leaving, actually leaving (hiding around a corner - she just continues running away), temporary restraint (she just kicks and flails and runs again). I can't get anything done when she's like this. I'm exhausted, and I have places I need to get to. (Was trying to get to school pickup this afternoon.)

Please does anyone have any advice

Tantruming for that long isn’t regular behaviour @hazelnutvanillalatte. Do you have any other concerns?

And how old is she exactly?
