Book about asking for help

My daughter (5) will not ask for help at school, probably not even if her life depended on it. She is shy, but will talk to friends and adults and her teachers quite happily and confidently - until she gets stuck with something and she shuts down. She won’t ask to go to the toilet if she’s about to have an accident, or tell the teacher if she is hurt in the playground etc etc.

This isn’t the case at home, or with extended family - I would say she behaves as any other child her age when out of school and is generally really well behaved. She’s not been told to stop asking for help or stop bothering anyone or anything like that. She is quite a deep thinker.

We are at a bit of a loss about how to handle this, as are her teachers. Can anyone recommend any techniques or books etc that might get it to click with her that it’s perfectly ok to ask for help when she needs it?
