2 year old poo withholding - help!

I have a dd who is 2 yrs 4 months - not potty trained. Since November we have had major issues with constipation (no idea what the trigger was). We took her to GP in November and they have prescribed Senna and lactulose. It seemed to help for a few weeks, poo was softer and smaller. DD does tend to have large, hard poos.

However things have deteriorated somewhat again. I think dd is withholding. Dd will be visibly straining (going bright red in the face, holding onto furniture) and there will be poo smears in her nappy. I think this is withholding rather than straining to go iyswim.

We haven't tried potty training yet - she will sometimes do a wee on it after her bath. Dd got quite upset when my mum showed her a potty she bought so we aren't pursuing it yet.

Has anyone got any advice? I'm pretty stressed and feel sad for dd
