Severe language delay in 4.3 year old

Hi my son's therapist recently mentioned that he has severe language delay.
He talks a lot but in one sentence or two or three words paired together. Also he has trouble comprehending. He does talk when he wants me to look at something. It's like he has the vocabulary and understanding of a 2.5 Yr old. He can't have a two way conversation with me. I'm worried. And wish to know when will he get better. We have just begun therapy. Had no clue about it since my eldest also was a late talker but he turned out fine at school. Is anyone here in the same boat? How did your child cope with it? My biggest fear is him not being able to make friends and be alone during playtime.

If the SLT is concerned, have they suggested a referral to a Paediatrician?
