Help 4 yr old son stool witholding

My son is 4yrs old and has been doing this ever since he has been potty trained. I have been to paediatric specialist who said it is psycholigical now as he can go he just chooses not to. The specialist has had him on movicol, lactulose, senna you name it but he has learnt how to control these or he will become explosive and ends up with a sore bottom. I am kind of at my wits end as i have tried all the reward charts, books for him and ive read loads of books too. He wears pullups all the time otherwise i can go through 10 pairs of pants a day where a little has come out and he has squeezed the rest back in. He will have no marks for a couple of days and then its constant until he goes at maybe day 12...unless ive given him any of the meds and then he is explosive. Ive made an appointment for specialist again but not for 2 months and a gp appointment to see if he can suggest another route in the interim.
Anyone got any advice?

Hi Op, I had something similar with my son. He developed a random fear of the toilet! He was happy to stand a do a wee bit not sit for a poop.
We tried all sorts of things, just like you. We had a "poo" specialist come and visit us at home and the only thing that made a difference was making him gradually sit on the toilet for a wee rather than stand up. So he just learn to relax on the loo and eventually he started going. But even then it must have taken another couple of months. I know it's not exactly the same situation but hopefully might help x
