Is this normal for a 3.5 year old?

DS3 is the 3rd of 4 boys. DS's 2 and 4 have disabilities. DS3 is hypermobile and wears piedro boots to support his ankles.

He's into everything and has the attention span of a goldfish. He is always trying to escape and when we're out he has to be in the buggy as if I let him out he will run off. He is always covered in bruises from climbing on things and falling off. I seem to be always telling him to stop, come here, don't touch etc.

I think part of it is he spends so much time at hospital appointments etc with DS4 and he's bored. When I manage to do things with him like soft play or even just playing with a bucket of water and toy boats in the garden he is really good but with 4-6 appointments a week for DS4 plus housework and clearing up when he's flooded the bathroom/tipped all the toys out etc there usually isn't time. I think he would benefit from preschool but pick up and drop off would clash with DS4's appointments and there isn't any preschools near DS1 and 2's school.

Any suggestions? DS1 went through a similar stage when DS2 was the same age DS4 is now but I don't think he was this bad or maybe I've blocked some of it out. DS3 has completely destroyed DH's model that it took him months to make today as well so no doubt I'll get it in the neck for not supervising DS3 properly. I do watch him 99% of the time but I still have to go to the loo sometimes.
