I've just been sent a questionnaire in lieu of the 18mth check as my HV has said the toddler check is being phased out here unless the parents or any care givers feel there is a problem.
It has three sections; physical, emotional and social and communication. Under physical it asks for weight and height (I don't know his weight as I haven't had him weighed since he was 1yr) It asks if he's walking/crawling. Also asks if I've noticed any problems with his mobility, his eyesight, his hearing or his eating.
Under emotional it asks if he's happy and if he's affectionate also something about his crying which I can't remember (sorry) Also about sleep patterns.
Under social & communication it asks stuff like do I think he understands me, does he communicate with me using language (it stated that this could be babble not just words) Does he gesture like waving, clapping & pointing.
Does he enjoy playing with his toys, can he recognise pictures of objects he knows in a book.
There was also a bit at the end which asked if there was anything I was worried about.
Sorry if this is long, hope it helps a little. I think it varies from area to area as I know the daughter of a friend of mine was asked to share her drink with the teddy bear and to comb her mummy's hair.
