Nearly 4 year old wetting himself

My ds will be four in September. He has been potty trained for a year and, except for the occasional accident, he has been dry. He still wears nappies at night and is dry most mornings. This week, he has had a poo in his nappy on waking 4 mornings. He has also: sat in the potty and weed with his trousers on; weed in the bath twice and weed on the sofa and sat in it for half an hour without telling me. I am totally at a loss as to what to do!

My first thought is possibly an infection, urine infection maybe? Has he complained about it hurting when he wees? Or has anything upset him/changed in his routine?

What does he say when you ask him what happened?

Thanks for your comment. It's not something I had considered before. He can't tell us why he is doing it but I have just asked him and he says it doesn't hurt. He says he forgot!
