Ok, my DS is awful getting to sleep at night - WWYD?

My DS is 7m old. During the day he goes to sleep fine in his cot. I put him down drowsy and he sleeps (but only half hour naps) At night he has a routine of 5-dinner, 5.45 bath, 6 bottle then down by 6.30. He will sleep for 20 min then wake up and have a full on tantrum if you don't pick him up and rock him back to sleep. This can take from 5-45 min. He will often wake again at 9 - same thing. I know people have posted the slow removal of yourself from room over a few days method and the shh and rub back but what do you do if you have a child that will scream till they make themselves sick? He has such a tantrum that he is not even aware which of us is in the room as seems to be in almost trance like state of screaming and doesn't calm down for ages whether sung to, rocked etc.
Do I leave him where he will end up sleeping on a soggy, snotty and potentially puke covered sheet or do I pick him up before he gets to this state?

With pick up put down you would rock up until he is relaxed and then stop so perhaps the first step may be to rock him for a while and then stop and just hold him whilst he still awake so he is not reliant on being rocked to sleep, then working on putting him in the cot awake rather than being held until asleep.

The point is not to let them cry/get upset and intervene before that point.
