Lack of awareness of pain and illness

How best would you teach DC's to recognise signs of illness in themselves?

After recent events with my DSS (aged8) it seems clear that he doesn't understand that pain is his body's way of telling him something is wrong, and changes to his toileting or vomiting are things he should tell an adult about!

It seems likely that he has been unwell with an upset tummy for a week - but hasn't told his mum or anyone at school and when DP asked him 'does his tummy hurt?' he said 'oh, yes' in a matter of fact tone of voice that suggested he was used to it :-(

Even as a toddler he displayed little distress when in pain - what (if anything) can we do to teach him that he doesn't have to tolerate illness stoically without complaint?!?
