Get him a doctor set and let him practise on his toys?

Never tell him something won't hurt when you know it will.

Tell him things will hurt a bit but will make him better.
Take a toy to the surgery and have the doctor do everything to the toy first.

We had this with DS aged 3. Our HV offered to give us a tour after hours, worked a treat! She had chocolate buttons on hand and stickers etc, he had a go with the equipment. Very helpful.

Some things we've found helpful were looking at doctor videos on youtube, books about doctors like this one, going with his older sister to observe, and playing with a doctor set.
