Almost 3yo DS, communication diffs, frustration and development

DS is almost 3 (next month) and has speech and communication difficulties. We have been chasing a 'diagnosis' and support for months, paid for private assessment but seems nobody has hit nail on the head yet about where his delay is stemming from.
He knows plenty of words but speaks in rehearsed soundbites, doesn't ever tell us he is tired/hungry/sick, doesn't really ask for things. Other people don't understand him at all.
He started potty training but regressed and now pees anywhere (tends to strip off as soon as he comes into the house) and won't keep nappy or pants on.
In the midst of all this, his little sister arrived 6 weeks ago and I know this can't help - I am really trying to be patient with him but his tantrums hard to deal with when I am exhausted from night feeding or baby is crying for food (is bf).
Today he threw my phone at DD and hit her head. I just didn;t know how to discipline him and cried my eyes out Sad.
I just don't know how best to help him, and how to be a good parent when everything is such a battle

And am worried that I spend so much time telling him off/trying to calm him down that I never get a chance to help him with his speech and development.

A speech therapist, who I assumed would be trained to spot any other issues. I am not sure I'd he has behavioual problems or is just a frustrated toddler! I just want to help him however I can
