Baby change of name and passport application

Hello!
I have recently changed my DD name. She is just 6 weeks old, so birth certificate was amended it using the space 17.
Now I need to apply for passport and I am unsure if I have to declare her old name. Anyone has any experience with this? I read that because her new name is in the birth certificate is not necessary but I am not sure 100%.

New name - that is her name, officially.

I have in fact recently made a name change for my son after he had been issued a passport in his original name. I’m now going to reapply to have a passport issued in his new name.

Hi! Many thanks for your reply, but my question was about if I need to declare her previous name. New name is in the birth certificate on the bottom so I am not sure what should I do. Probably will need to call passport advice line.

I have recently applied for my daughter's first passport, during the application it asks has if this person has ever been known by another name, I said yes and wrote what it was. When I sent her birth certificate off as part of the application, I wrote a note explaining the situation so there was no confusion. I had no problems and her passport was complete within two weeks. Hope this helps. X
