to remind her about the forms she took to fill in for me cos she didnt have her reg number on her (milk tokens, health in pregnancy grant) that she said she would definately return by today so i could get them sent off and sorted?

When are you next seeing her? If its reasonably soon you could just ask her then. If its a few weeks away then I would text.
