DWP do not return original documents. I would send a scan or photo of the passport. My relative has just applied for a supercession of her DLA and the letter said not to send originals. She has sent a copy of letter from her GP.

You can send his full birth certificate.

I sent an expired passport it came back 2 days later, a letter from the school confirming he was on role, a copy of the letter with his national insurance number and a bank statement
