Hairs matted, often wears dirty clothes, has had vomit in their bathroom tha has now grown mould on it for two weeks, lots of rubbish in their room and out of date food and even mouldy clothing and other items

Nobody would choose to live in those conditions if all was well mentally.

They are obviously neglecting themselves and their home. I would definitely assume something was wrong.

I would say this person has serious issues and being lazy isn't one of them.
They need help no one would willingly live like that and in those conditions.

Lazy is putting off doing the laundry because you can't be arsed, or having a sandwich because cooking is too much effort.

Leaving vomit until it's gone mouldy, neglecting personal care until your hair is matted, etc. is sign of a problem, whether mental, physical or both. It's well into 'not coping' territory.
