to extend my sick note

I have been signed off work for 6 weeks and my next sick note runs finishes this week so I am due back in work on Tuesday. I am feeling better but still not at 100%
I have had no contact from my manager regarding my return, he was meant to do my first absence review but he cancelled and I have not heard anything from him. It would be good to have a phased return and also a referral to occupational health but considering i am due back on Tuesday this looks very unlikely.
AIBU to extend my sick note?

Definitely not unreasonable, but if you are feeling better then maybe it’s worth contacting your workplace tomorrow to discuss your return or ask to arrange a meeting later in the week to discuss a phased return?

Have you told them of your intentions?
Some managers are reluctant to disturb staff on sick leave, fearing it will be seen as pressure. I think you need to communicate, especially if it is a small company.

If you are still unwell and don't feel that you are able to go back to work then you need to speak to your gp for another note. You will have to let work know what you intend to do and how much more time off you will be having. Depending on the nature of your illness/ absence they may then organise an occupational health phone call to discuss the situation/ your health etc with you.

It depends what it is but lots of people go in when they are not feeling 100%.

I haven't been able to tell my manager how long I would need off as its an ongoing condition and I have to wait for the medication to work which its is slowly starting to do. I feel bad that I haven't been able to tell him how long I would be off but it is impossible to tell
I would like OH referral as I have a condition that is classed as a disability under the equality act and I have explained this to my manager.
I worry about going back too soon and having to go off sick again
