Wrong medicine dose prescribed…again

My lo has been wrongly prescribed a medication dose by GP. LO is 1yo and prescribed an adult dose which is 4x above what LO should have. Last time we only realised because we gave it to LO and LO started vomiting a lot. We rung the pharmacy who then made us realise what had happened. I complained to the GP and they said it would be considered a serious incident, it shouldn’t have happened etc… I said fine and left it there. I was pretty mad as my LO was in hospital earlier this year with gastritis and a suspected bleed.. LO had to have a blood transfusion and a lot of investigations so I felt that for them to prescribe LO such a high dose even by accident was pretty bad considering LOs history.. but they were apologetic so I decided to leave it there.

Then they’ve literally made the same error about a month later.. my LO is one.. I feel like I’m having to double check their work and just don’t trust them anymore . I’ve complained again but I’m just wondering if any one has been through a similar situation. And what they did. Is there anything I can do so this doesn’t happen again to my child or a different child?

That's very worrying. I believe the GP would be obliged to report it as a serious adverse incident, and reporting the same mistake twice in a short space of time, especially with the same patient should surely raise red flags about their competence (or excessive workload which is compromising their ability to do their job effectively).

I wonder if birth date is on the prescription... then it could be used as a double check for the pharmacy to check against dosage. Not that that excuses what's happened as that is shocking!
I'd be wanting something in writing from the doctors to document that this has happened again.
