I have a friend who has been in a relationship with a guy for a long period of time, 10 years +, in that time they have never been on holiday for longer than 2 nights together, she also has never met his family.

They have recently had a baby together and still only spend 2 nights a week together. He comes from a culture that is known for arranged marriages, he also travels there for a few weeks every year, AIBU to suspect that he could have another family?

Nothing you can do here OP but she's obviously happy with her head firmly buried in the sand. It's the poor child I feel sorry for.

Does it matter? Your friend is happy and has chosen to have a baby with this guy so nothing more to be said really...

It’s her mother I feel sorry for as she is now having to be the second parent! She’s the type of person who can’t take any criticism and fly’s off the handle so don’t feel I can even broach the subject. When their relationship first started he was working in his takeaway most nights and she did a lot of ferrying to activities for her older children so it wasn’t as weird. He now doesn’t have the takeaway and she’s not being a mum taxi but the relationship does not seem to have evolved at all, I just can’t fathom bringing a child into that situation

Dodgy would imply that she’s being misled. She isn’t. Unless you think she’s completely stupid, she knows they don’t have a real relationship and that there’s no prospect of one suddenly materialising after over a decade. She’s chosen to stick with this casual arrangement.
