Dyslexia assessment for work access arrangements

Does anyone have experience of getting an assessment for dyslexia so that they can get assistance with their work equipment/environment?

When I worked for a Local Authority and such assessments were arranged through the Occupational Health Unit and were undertaken by independent assessors. The assessment report recommendations informed equipment requirements and workplace adjustments. Process worked well for a couple of people I managed, though facilitating a quiet workspace with few distractions in a large open plan office was a bit of a challenge.
