Can’t cope with these symptoms

I was extremely undecided about posting this incase I seemed like a troll, but MN can verify that I’m not and I’m a regular non-troll poster. I really need some help or advice please and can’t cope with this anymore.

I’ve got a problem where I get intense anal itching and pain- it’s like a burning/searing pain and the itch is unbearable. It’s not a fissure or pile (I’ve been to the doctors already). I have IBS and I usually get diarrhoea from it so it’s not from constipation. It’s usually worse after passing a bowel movement and also in the evening and when I am out walking at night, I end up in tears because the pain and itch is absolutely horrendous. I’m definitely clean there but yet I end up with marks on my underwear as if I’m not- but I 100% am and have tried wipes, wet toilet paper, all sorts and I’m definitely clean.

When I’m passing bowel movements, it stings and burns so much and it feels like I’m passing battery acid so I don’t know if it’s caused by that

Or I don’t know if it’s caused by irritated skin on the area, but I’ve tried changing my underwear material and type and I’ve not changed washing detergent or softeners recently, but I’ve changed them to a hypoallergenic one now just in case but none of that has helped. Anusol helps a bit but it’s still bad and it’s leaving me in floods of years everynight.

Whatever it is that’s causing this burning/itching/leaking it’s absolutely destroying my life and I can’t cope with it anymore. Once again I’m going to bed in tears with it all, it burns to walk and to sit down and I’m fed to with being so uncomfortable. I’m in my 20s so it’s not even an age thing either

GP - sounds like either piles or anal fissures

I’ve been to the gp and also had a camera test and they said I don’t have any fissures or piles x

Back to the doctors and don’t be fobbed off, tell them how much it affects your life.

what medications have your doctors tried out for you?

i wonder if it’s food allergy related - does what you eat make it better or worse at all?

Definitely needs medical attention if it’s that bad

I was also going to suggest a food/symptom diary to see if you can identify something in your diet which is causing it

I have no idea but would it be worth trying a barrier cream, like drapolene, before and after a bowel movement?

Have you tried eczema cream? It might be bad eczema. I speak from experience.

Can you relate it to anything you eat? I'm guessing you already avoid anything spicy?
But perhaps it could be anything too acidic? Some fruits can be too harsh on the stomach.
Maybe check out the FODMAP diet and see if any of that resonates.

Also, are your stools soft or are they difficult to pass?

Definitely not worms? If not, you'll need to look at your diet. I have gallstones and the tiniest bit of spicy food has me pooing what feels like lava for half the day.

I am allergic to anusol and oh my god, the itching! Try germaloids cream and suppositories for a few days instead and if no improvement, back to the gp.

Pruritus ani. I have this too. Makes me miserable.

Back to the GP. It's great they've ruled out some things but they can't just stop there.

I'd suggest a barrier cream if you haven't tried it, any nappy rash sort will do.

As others have said it's also worth thinking about food allergies. It's so tricky to pinpoint when things are already bad but if you can go super strict on least likely foir groups and see if it improves then you'd know.

Op, have you had stool samples taken? Also, anal STI testing?

Internal 'skin' condition also sounds plausible. I suggest you lubricate lower rectum/anal passage with 50::50 ointment before motion to see if it provides some relief and ease.

i'd bypass the GP and go see a specialist (privately) if you can afford it.

Buy some frozen Brussels sprouts. Take one out wrap it in cling film and Pop it inside your cheeks against your anus. That should break the itching cycle. Have you thought about thrush.

As previous poster said have you tried eczema cream, nappy rash cream or even savlon. Def go back to the GP though.

Angelic toilet paper gel
Canesten hydrocortisone
Portable bidet spray...

Have you tried drinking more fluids, eating more fibre (maybe try some prune juice if you are near a toilet for the next day? ). Have you taken worm tablets? Maybe something to meat area heal, such as Sudocrem? Good luck.

Food diary is an important step. My dh changed his diet a couple of times and found a very low carb diet worked for him. He had developed itching so bad he was scratching in his sleep, had diarrhea and had large areas of scabs around his anus. This has all resolved including brown patches that developed around his neck and chest. I can't eat seeds or lentils. Eggs are a bit dodgy as well. It's a process of elimination and difficulties can develop at any age.

Are you sure you don’t now have internal piles , as when I had them no matter how much I cleaned my bum I always felt dirty as the poop hides around the piles then seeps out with body movement. I don’t like anusol but germaloids suppository’s are like a gift from god. I also get a medical glove fill it with water freeze it and stick the fingers in my bum. God I’m so glad this is anonymous lol

First of all, I’m so so sorry you’re going through this. I posted myself on this board about bathroom problems about a month ago and a lot of people were really kind. But this right here says to me that there’s some kind of problem. If you’re ABSOLUTELY sure you’re clean, then perhaps you’re suffering some kind of minimal (not obvious enough to leave large wet spots) but obviously very painful
anal leakage? If so, that could leave you with constant itching and inflammation down there, which would be incredibly sore. It would be like the horrible burning/soreness you get after you’ve had a very terrible few days of diarrhoea, because: your bum is very sore, your stool may be very acidic, etc. Or it could be, like many posters have said, that something is off balance in your stool, maybe from a dietary intolerance, and this is causing more burning.

Perhaps talk to your doctor about trying something that will cut down on acid and see if it make any difference? People sometimes only think of surplus acid as it affects your upper digestive system, but obviously if you have enough, it can cause issues with your lower digestive system (either because it affects bile production, or just the acidic level, etc). I don’t pretend to be an expert at all, but try gently cleaning completely outside and inside as far as you can reach (safely reach! Don’t push!) and then applying a barrier cream BEFORE using the loo is a good idea to try for starters, and please, push your doctor to take a stool sample and help determine why going is hurting you so much especially since they looked and did a camera test and told you that there’s no “sign” anything down there is wrong - pile, fissures, inflammation, scabs, broken skin, etc.
