If you've had your driving license taken off you because of a medical reason ( ie strokes poor eyesight etc ) how do you get it back ? Is it that they just give it back after a certain time or do you need permission from doctor before you get your license back ie a letter from them to dvla ? Anyone know how it works ?

It depends on a number of factors to do with the medical condition and how you gave up your licence - no one can really say for certain aside from the DVLA in conjunction with advice from your GP.
