Give your GP a call, they can decide if you need a referral for it to be investigated further. Probably nothing but always best to get it checked out.

Is it the actual nipple or the breast itself? And how long has it been like this?

You should make an appointment with your GP. It could be nothing, it could be serious. Your GP is best placed to make that assessment.
