Depends on the team really.

In my team in order to be referred you would need to have registered with a gp in the correct area. We can then process your referral. Your current team would decided if its routine or urgent. Most for our team is then a 28 day referral time, you would then be assessed/ accepted. If you are just waiting for a dr appt then that might be smoother, if you need a new care coordinator then there may be a waiting list.

Your current team would be responsible for your care until its fully handed over and might remain so until you have a new care co or might discharge you once your referral has been sent.

Its very dependent on the accepting teams availability and your needs
