All you need to do is contact your local hospital (or GP depending on what records you want) and make an Access to Health Records application. The PALS department can help you.
They will also inform you of the charge as it isn’t free

You can request to see them at your surgery.

Also, if you apply for an online account, you can access your medical records there...

Contact your GP surgery. You will probably need to make the request in writing. Legally, they have 30 days to get them back to you (and they are allowed to take longer if they really need to).

Bear in mind that this will only give you the consultation notes for GP appointments and letters they have received from hospital consultants (so you won’t receive consultation notes for hospital appointments).

You will probably need to show photo ID when you collect the records from the surgery.

(If your surgery has online access, you’ll be able to access your records on there, but only those since the early 2000s when GP surgeries switched to online notes)

You won’t need to pay, but you may need to provide a memory stick if you want a digital copy (as we’re not allowed to email medical records to non-NHS emails).

ViaSacra I requested my records a while back and was sent a few attachments by email from the administrator. I wasn't happy about this. Did they breach data protection do you know?

Sorry OP for butting in on your thread! I was told it's free now. Subject Access Request. They can only charge in certain circumstances - and the fee shouldn't be excessive.

Ask your surgery for Patient Access, sign form at the reception, they will give you access code there and then, then go home and download one of the APPs on your phone. I have Evergreen, but there are loads of others. You will see all your medical records there.

@itsbedtimenow - they can send to non-secure email addresses (gmail, hotmail etc) as long as the person whose records they are provided informed consent re this
