My poor poor arsehole (Tmi galore)

I have ulcerative colitis and chronic constipation so it's all or nothing. I also have internal piles which have been unsuccessfully operated on, which bleed every day. To add to the above, I now have an ulcerated bloody anus Sad

It's a constant bloody battle. Blood, mucus, pain. Is it the colitis or the piles or the constipation? Are these ulcers colitis related or from too much wiping due to blood and mucus??

I know you can't answer and I need to show my arse to yet another doctor but aibu to want a bloody break from my stupid fucking defective arsehole?? Angry

@Persimmonn one quick thought is - I think you can get a condition (called "anismus") where the coordination of your bottom muscles gets messed up so that when you try to go, you inadvertently tighten and close the opening to your bottom. I think it makes everything feel tighter and obstructed when you try to go and can result from knowing that going will hurt. Maybe that could be a possibility?

wigglesniggles · 21/04/2019 00:19

Agh its such a basic function isn't it? Sorry for your bottom troubles.

DF had the worst case of piles his doctor ever saw. I remember meeting him at the airport and he could hardly walk. DM had to give him sanitary towels. I smirked but got my karma years later. I had severe constipation after surgery and was hours away from an enema when I did a foot long poo which gave me piles. That was the thing I was most upset about when I left hospital, not the unplanned emergency surgery and life changing diagnosis. No, 30s and piles.

OP why are you saying you are “symptom free” and that’s why you weaned off medication? You know that ulcerative colitis is a genetic autoimmune disease don’t you? It doesn’t just go away. You don’t sound symptom free at all. You need to go to a geneticist or immunologist, stop eating gluten, stop drinking alcohol (if you consume these things) and go from there.

Persimmonn · 21/04/2019 09:13

rachb that sounds like what happens. I’ll have to read about that later, thank you!

AnnaMagnani · 21/04/2019 10:08

Having thought about this overnight, you need to go back to your gastroenterologist - or get re-referred if you don't have follow up.

You aren't symptom free from your inflammatory bowel disease - the ulcers are a symptom as is the constipation and the combination means you may actually have Crohn's as @AlunWynsKnee suggests.

You're absolutely right, I'm not at all symptom free. To be honest though, the first question they ask is how many BM per day and if it's not a lot then they're not interested. I'll make an appointment though.

You need to go back to the doctors. If the suppositories aren't helping you go, have you tried an enema?
You may need more medication, mesalazine suppositories or foam enema may be your best bet if the ulceration is very low down.

Op, I'm a nurse and would advise you to wipe your bum with Baby Oil or even olive oil, as opposed to wiping with paper or wipes, which tear the skin more.
Any barrier cream should bring some relief, but you do need to be seen by a Doctor asap.
I've a son with severe UC, and if he was suffering like this I would't hesitate to say get a stoma!

Divgirl2 · 21/04/2019 21:45

DP has UC amongst other things and he is mesalazine long term to prevent flare ups. It smells like stale beer. He's also on azathioprine (although that was added in post liver transplant).

He can spend between 2-3 hours on the toilet on an average day but other than tenesmus he doesn't really have any symptoms, and hasn't for as long as I've known him.

Definitely ask to go back to your gastro. Are you having annual colonoscopies?

NaiceToMeetYou · 21/04/2019 21:59

It's almost 2 years since my diagnosis and first and my flare up. It lasted a month and I lost 10kg in weight. I had a colonoscopy to diagnose, then 3 monthly checkups for a year. I was then changed to 6 monthly and was declared in remission at my last and discharged.
I've been numerous times regarding the constipation and mucus/blood. Gp says its UC related and refers me to gastro, they conclude it's constipation/overflow/irritated piles and I'm given laxido.
Not had a colonoscopy since dx.

I've been in remission for over a year, but still have to go every six months. Even had an "urgent" flex sig yesterday as my bloods and stool sample showed high inflammation. There was no sign of anything. The last consultant didn't listen to a thing I said, hope I have him for my next appointment so I can say ha! Told you so!
Anyway, you need to get the constipation sorted, it's not nice to not go and so uncomfortable if you're at all inflamed.

PinkGlitter123 · 22/04/2019 09:08

I also have six month checks and am on mesalasine as a maintenance drug. Also had to use enemas to get to the ulcers that the oral meds couldn't.
Best of luck OP, I really do sympathise x
