To cancel for a second time

I am going through a hard time at the minute, and had arranged to meet up with someone to chat about it. However, on the day I was absolutely exhausted and feeling generally unwell (this has been coming and going unpredictably for about four months, I had been to the GP several times prior to asking the person for help and had blood tests which came back normal) and had to reschedule for about two weeks later. The rescheduled date is now coming up, obviously I had hoped to be feeling better, but I'm not and am going to the GP again tomorrow. The person has seen me at a distance a couple of times since, but in a situation where I just had to sit on a chair and not actively participate. At the minute I'm really not feeling up to going into detail about my personal life, but the person is doing me a favour and I really appreciate their help, which I asked for (while I was feeling okay). I absolutely hate cancelling things and not doing something I said I would do, once never mind twice. What should I do?
