AIBU to not feel ready to go back to work?

I’ve been off work for nearly 3 weeks due to a ME/CFS relapse set off by the flu. My doctors note is due to finish this week, but I don’t feel ready to go back - I’m still exhausted, alternating an ‘active’ day with a rest day, and am worried I’d make a mistake at work in this condition. Currently in the lucky position that I don’t need to worry about bills. DP is supportive of me either way, but my DM is convinced that if I don’t go back now, I won’t want to (which is not the case at all, my job may be stressful but I love it & have spent a long time getting to my current position!)
I think extending the doctors note for another week is reasonable, as there is then a pre-booked week away with DP’s family that will be very relaxing & should set me up well for a return - my symptoms are always better in warmer weather, which is probably why I’m still struggling to shake this one! DM feels I should go back this week & get on with it, as I’ve got another week off coming up. AIBU to want to extend the note & return after the hol, or should I try my best to stick out the week back at work before being off again?

Compromise. Do a half week if you can.

Going to look into a phased return with work if it’s doable (not really the sort of place you can do 1/2 days, sadly) but have an appointment with a specialist later this week to see if there is anything they can recommend
