AIBU to think I could loose this job offer?

After surviving 2 years in a bad job, Dr signed me off with stress for 2 months. At the same time, I looked into why I couldn't manage the job, and an OH assessment revealed I'm dispraxic. Lots of recommendations were made, but my manager said none of the 'reasonable adjustments required' could be made.
I've now found the perfect job (less hours and less stress) but the offer is conditional on acceptance of the completed heath questionnaire. Surely when the new employer sees I've had 2 months off with stress, they will withdraw the offer.

Not necessarily. The hours / distance would be an improvement on your stress levels. You may be asked to an interview with an occupational therapist or they may seek to contact your dr.

To try and put your mind at rest I don't think that will be the case at all. I also think it would be illegal (emphasis on think) to retract the offer on that basis. I am not sure why healthy questionnaires are carried out, but I assume it is so the company can be proactive in helping with any potential problems.

I took voluntary retirement from a very high profile job when my DH died.

I then applied for and received a job offer (well within my capability but without travelling). I just had to agree that my new employer could ask for medical references from my health professionals.

I am sure that you will be okay.

I had six weeks off with stress in May and got a new job in September. No one questioned the sickness. I filled in OH form and the question was 'have you been off ill for more than 3 months due to work related illness ?' I answered no and nothing came of it!

The OH form usually only goes to the OH team, not the managers. It's only if you disclose the days off and agree to the OH disclosing it that it will get back to them.
However some of it depends on the company. They could remove the job offer if they could prove that supporting you in the role is so difficult and you are not fit for the role....but it's unlikely. The fact that you've gone for a part time job shows you are managing the stress and you have a diagnosis.
If you had a nice interaction with the manager at interview, give them a call and give them a heads up.

I work in a role where I need a full disclosure and medical questionnaire etc. I have been off on a couple of occasions due to stress (abusive exp, police involvement). My medical questionnaire was done so that they were aware of any health issues I may have and so that if i was off on commencing the role, there was an understanding of why I was off).

I appreciate this is my situation and not yours but just be honest. Best of luck OP
