AIBU to now quit my job (light hearted but mortified)

So - my boss (male, 35 years older) - is scheduled to do an observation of my performance on my job - he is meant to observe a session I am running. It involves a form.

I need to part fill in the form and send the rest to him before the session he will observe and he fills in the rest.

Simple enough?

I have just emailed it to him saying -

"Hi John

Here's the form for tomorrow's session. I've filled 1-4 in. The rest of the stuff there - they are still my thongs but you'd need to fill them up.

Best, Focus"
