We've done Canada (west coast), Guatemala and Honduras (not safe!!) and Dominican Republic (but definitely NOT in an all-inclusive resort!) with our dc.

They all want to go back to all of them (are 15,13,11).

We've been to Vietnam a couple of times with kids but not sure what the weather is like in the summer as have only been March/April. They loved it. I prefer it to Thailand.

Malayasia rainy season starts a bit later so weather should be OK, English is widely spoken being a former colony so no problem accessing English speaking Dr's and pharmacists if you catch any mild illnesses.

Less hawkers and touts on beaches trying to sell you accommodation and excursions. You can fly into Singapore as it's on more international routes and spend a couple of days at Universal studios but Singapore can be quite pricey. Rather than a month in one place you could do a few countries as AirAsia flies to quite a few destinations across S.E Asia

US/Canada - some amazing campsites so you can keep cost down once you've paid for the flights.
