Currently on maternity leave. Have been offered perfect career-type job elsewhere and I want to take the job, but I don't want to leave the kids!

It sounds like a good opportunity so I would take it. It might be hard at first but you and the children will quickly adapt.

Kids are 4 and 11 months, and it's 3 full days per week!

That's the thing, DH travels quite a bit so the majority of the ferrying to/from nursery will be done by me!
