To think this is unacceptable?

My setting leader (am 1-1 with a child in a playgroup) filled in a risk assessment form. Well I took this child?s file to sort in the holiday and I actually read the thing and it was full of spelling mistakes including the child?s name.

AIBU to think this is really bad practice? What can I do?

well it would be bad practice to have spellings errors in an official document which I presume the risk assesment is.

You can only point it out to her really, but could cause some embarassment on both sides doing that.

Well it is embarrassing. The form is basically aform to state the risks in certain situations but it looks so bad I have done a new one but you can?t just throw the thing away.
